Based on interviews, facility did not ensure to have child signed out which poses a potential Health, Safety, and Personal Rights risk to persons in care. 8 POC is cleared today. Memo was sent December 18, 2023 and staff received training on the same day.
Based on results received from the certified water samplers, the facility exceeded 5.5 ppb of lead in their water source. Once received, the facility took immediate and proper actions to remediate the fixtures. This poses a potential health, safety, or personal rights risk to persons in care.
