Based on LPA observation the licensee did not comply with the section cited above in that at 7:45AM LPA observed school age children comingled in a preschool classroom with 14 preschool and 2 toddlers which poses an immediate health, safety or personal rights risk to persons in care.

Facility Representative will submit proof of repair (invoice/pictures) to the department. Cleared during visit- photo obtained of repair. Based on direct observation and interviews, the facility did not comply with the section cited above in that a play structure has a broken bolt and is not anchored to the ground which poses a potential health, safety or personal rights risk to persons in care. 8
