101212 Reporting Requirements (b) The name of the child care center director, and any fully qualified teacher(s) designated to act in the child care center director's absence, shall be reported to the Department within 10 days of a change of child care center director or designee(s). 1 Regional Manager will submit a complete Director packet to CCL for review and plan of correction by 10/26/2023. Based on interview, the facility did not report change of director or submit a completed director packet to Community Care Licensing (CCL) for review within 10 days of a change of director. This poses a potential risk to the health, safety, and personal rights of children in care. 8

Based on record review and interviews, facility staff failed to inform parents of an injury that a child sustained while in care. This poses a potential risk to the health, safety, and personal rights of the children in care.

Based on observation, interviews and record reviews, a preschool child was left unattended in the preschool playground for approximately 5 minutes on 09/01/21. This poses an immediate threat to health and safety of children in care. 8 According to AB 633, parents must be provided with this report which contains this Type A deficiency for the next 12 months and copy of signed acknowledgement form must be kept in each child's file. LPA advised Licensee that an Office Meeting will be scheduled at a later date. Written notification of the office meeting will be sent to Licensee via mail.
