LPA Valdez Santana and Jung explained to the Director the difference between the LIC9182 and the LIC9163. Director stated she forgot to call the Licensing Office to get Nicole Lockmer associated. The correct form was submitted to Licensing office during visit and Nicole is now associated to facility. Teacher Nicole Lockmer began employment at the facilty in May 2023 and has been present at the facility without being associated to the facility. Based on observations by LPA during the 1 The director stated that they will draft a plan to address the lack of supervision, with all staff at the facility. Proof of this and the plan will be emailed to LPA by 8/10/22. inspection on 8/9/22, it was determined that the staff do not properly supervise children in care. LPA observed 14 preschool children playing outside with out the visual supervision of a staff for approximately 4 minutes. This poses an immediate threat to the children's health, safety, and personal rights.

One of the staff files was not complete and available for review, this staff is a substitute and present due to 2 regular staff being out for the day.
