Based on observation, the licensee did not comply with the section cited above in that LPA observed a School Age child in the Infant classroom, which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on record review, Staff 1 (S1) had expired Mandated Child Abuse Reporting (AB1207) training on 01/2022 which posed a potential health, safety or personal rights risk to persons in care.
Based on observation, there were infant bottles, sipping cups and baby fruit cup in the infant refrigerator but the bottles, sipping cups and baby fruit cut were not label with the infants name and date which posed a potential health, safety or personal rights risk to persons in care.
During LPAs outdoor tour inspection, LPAs observed that the infant playground has a cover for shaded but the tarp that provide the shade for the infant was too largre and did not cover the pole correctly. The tarp was lose and not tight. The tarp was light and did not provided enough shade for the infant which posed a potential health, safety or personal rights risk to persons in care.
