Based on LPA's interviews and review of the 1 The director stated to LPA that they addressed the incident with staff involved, and met individually with staff #2. Director emailed all lead teachers and met with them in person, to discuss the incident, importance of supervision, and has implemented a new procedure to check in and children on a white board that is hand held by staff when incident, it was determined that a child #1 was left on the outdoor play are for approximately 2 to 3 minutes. This presents an immediate health and safety risk to children in care. 8 coming inside and outside. Proof of this action towards correcting this violation and communicating this to staff has been provided to LPA during this inspection. LPA also provided director a brochure assisting the facility with Supervision techniques and improving overall supervision.
