Based on interviews conducted and documents reviewed, it was confirmed that Staff 1 pulled Child 1 by the hair and tapped C3 on their hand. This poses an immediate risk to the health and safety of children in care.
Based on interviews and record review an incident that occured on 1/20/26 reporting Staff 1 pulled child's hair and tapped C3 on the hand was not reported to the department until 1/26/26 which poses a potential risk to the health and safety of children in care.
Based on interview, and record review the Licensee did not ensure that Child 1's personal rights were protected by Staff 1 taking a picture of them using the restroom. This poses a potential risk to the personal rights of chidlren in care.

This requirement was not met as evidence by: 1 FR states water sources with ALE have been remediated and retested on 03/28/2023 and no longer have ALE. LPA received documents with updated results in email clearing deficiency on this date. Based on record review the Licensee did not ensure water sources at facility to meet lead requirements. Lead test results received showed that multiple water sources has an action level exceedance. This poses a potential health, safety, and personal rights risk to children in care. 8
