Based on observation, the licensee did not comply with the section cited above. During today's inspection, LPA observed solid waste in a trash can that does not have a tightfitting cover which poses a potential health, safety or personal rights risk to persons in care.
Based on observation, the licensee did not comply with the section cited above. The facility has a large metal dome structrure in the playground that is not anchored. LPA was able to lift the dome off the ground which poses a potential health, safety or personal rights risk to persons in care.
Based on record review, the licensee did not comply with the section cited above. Several staff members did not have vaccination against influenza, and one staff member did not immunization records on file to verify that he or she has been immunized against pertussis and measles which poses a potential health, safety or personal rights risk to persons in care.
Based on record review, the licensee did not comply with the section cited above. Staff members completed mandated reporter training in January 2019 and has yet to complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training which poses a potential health, safety or personal rights risk to persons in care.
