(1) No child(ren) shall be left without the supervision of a teacher...Supervision shall include visual observation. Per director, staff has been retrained around active supervision and completing additional headcounts/roll call measures. Director has also re-examined staff’s schedule to ensure additional support during transitions. Staff will be completing headcount/roll call that includes name-based tracking at regular intervals, upon arrival, before and after transitions (e.g outdoor play, bathroom breaks, and indoors). This requirement is not met as evidence by, per LPA Laureano's document review and interviews, facility left a child unattended for for a brief period of time on 10/29/25, which poses an immediate health and safety risks to persons in care. Staff will visually confirm each child's presence and check of their names as our standard protocol. Citation was Immediately corrected. Staff development was completed 11/20/2025.

101223 Personal Rights (a) The licensee shall ensure that each child is accorded the following personal rights: (2) To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. 1 Director dicussed Facility's Procedures for All Teachers & Assistants on 8/12/2024. All procedures are given to all new hires. Procedures have been modified to higlight the importance of storing personal belognings . This requirement is not met as evidence by, per LPA Laureano's document review and interviews, child 1 had access to multivitamins that were found in the classroom, which poses an immediate health and safety risks to persons in care. Facility provided LPA with copies of procedures.

Director met with staff to discuss incident and retrained staff on utilizing the Teacher Child Ratio Log that all teachers utilize. Facility has also implemented different check point where staff confirms children in attendance. This requirement is not met as evidence by, per LPA Laureano's document review and interviews, facility left a child unattended for approximately 13 minutes on 6/12/24, which poses an immediate health and safety risks to persons in care. Director agrees to train staff on visional and auditory supervision during the use of the preschool yard going over potential scenarios and will submit meeting points to LPA via email.

Director will email LPA a copy of Based on observation,interviews, and review of lead water testing samples taken on 10/27/2023 revealed that levels of lead were beyond allowable amount deemed by the State of California- 1 water outlet/faucet (outdoor pathway between classroom D and E).This poses a potential health and safety risk to children in care. 8 parent notification letter via email by 12/14/2023. Director will send LPA a copy of schedule removal.
