Based on record review, the licensee did not comply with the section cited above in 1 out 2 staff does not have criminal record clearance on file which poses an immediate health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in 1 out of 2 does not have proof of Mandated reporter training on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in 2 out of 8 children missing admissions agreement and 5 out of 8 children missing required licensing forms which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on interview and record review, the licensee did not comply with the section cited above in 2 out of 2 staff files which did not contain the completed mandated reporter training. The Director did not have the child care provider section completed and the Teacher present had not completed the training since 2018 which poses a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in 1 out of 6 children present which did not have a completed physician's report. Child was enrolled in April 2022 which poses a potential health, safety or personal rights risk to persons in care.

During records review, it was determined that Director's CPR/First Aid was issued by the American Safety & Health Institute. LPA determined that the course is not EMSA Certified as it does not have the EMSA Pediatric First Aid/CPR stickers. Discussion was held with Director regarding the importance of having a valid Pediatric CPR/First Aid.
