St. Vincent Ferrer Preschool and Day Care

Based on record review two out of six staff files (S1 & S5) were missing proof of immunization against pertussis and measles which poses a potential health, safety or personal rights risk to persons in care.

Based on record review, the facility did not comply with the section cited above, 4 out of 6 staff (S1-S3 & S5) did not have Health Screen documents which poses a potential health, safety or personal rights risk to persons in care.

Based on observation and record review, the facility did not comply with the section cited above, LPAs reviewed that incidental medical medication was stored at the facility for four children, however there were no incidental medical services plan developed for three of the four children, which poses a potential health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above in in which two children had medication not stored in accordance with the requirements which poses a potential health, safety or personal rights risk to persons in care.

Based on record review, the facility had 1 faucet that exceeded that allowable levels of 1 Center Director has made the faucet temporarily inaccessible by placing a plastic bag and posting an out of order and do not use signs on the fountain, and Director intends to shut off the water to the fountain to prevent accidental use, and children in care are receiving drinking water by using cups to retrieve water lead in the water. This is a potential health and safety risk to children in care. from the kitchen faucet. Director stated the fountain would be replaced and retested by POC date and will submit plans regarding how that will be accomplished by 07/22/22.
