The facility has made the faucet inaccessible to children and stopped using water from it. Teacher Directorr Leslie states that 21 day flush shall be completed 10/2/25 and retesting on 10/3/25. After water testing was completed, the facility had one faucet (A) located in the kitchen sink with an Action Level Exceedance of 6.1 8 The faclity will submit lead water testing results for faucet "A" once completed to LPA via email by 10/17/25

The facility has already made a plan of correction and has updated the diaper changing chart to include the names of the children who need diaper changes, teacher responsible for changing, and the number of times the child has been changed. The Licensee did not provide safe and healthful accommodations for a child who was sent home with a soaked diaper, which poses a potential risk to the health, safety, and personal rights of children in care. 8 Plan of correction has already been completed for the facility and diaper changing chart is now being utilized. Deficiency has been cleared.

cleaning solution bottle was left outside accessible to children and one child was sprayed with diluted bleach solution. 1 Licensee removed cleaning solution that was accessible to children and placed back into inaccessible location.
