Multiple teachers are to sign off on document daily. Based on confidential interviews, interviews disclosed a gate was left open by parents and C1 was able to walk outside of the playground area and was found by a parent. 8

101700.3 (b) (1) California Lead Action Level at Child Care Centers...A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. This requirement was not met as evidence by: 1 Director Roxanne Walker advised when she received the results on 12/23/22, the faucet was immediately placed on 'Out of Service." On 01/12/23, the faucet in Preschool "C" Classroom was replaced. During the out of service period, the facility used a water pitcher with water from an approved water Water testing results taken on 11/17/2022 with an Action Level Exceedance of 12 which is higher than the allowable limit. The faucet is located in Preschool "C" Room. This poses a potential health, safety, or personal rights risk to the children in care. 8 faucet in another room, and the children were washing their hands in the restroom. The new faucet was retested on 02/03/23, and the satisfactory results were received on 02/16/23, where report indicates faucet tested under action level exceedance. LPA received a coy of new report.
