Based on observation, the licensee did not comply with the section cited above in the playground with climbing equipment has cobwebs on the play structure and in the toys inside the bucket by back fence, the smaller water faucet has bird feces in the sink at time of inspection, children playing on playground prior to being cleaned. This poses a potential, health, safety or personal rights risk to persons in care.

This requirement was not met as evidence by: 1 Director agrees to submit results of the water sample test that is scheduled to occur on 01/21/2023, director will submit updates to the Department until water test results under Action Level excedance. Based on water testing results, interviews and records review, facility tested over the Action Level Exceedance, water outlet (located in kitchen) labeled A which is used for drinking water for washing dishes. This poses a potential health, safety, or personal rights risk to the children in care. 8
