Based on interviews the licensee did not ensure that care and supervision was provided to children during naptime due to the Director sleeping. This poses an immediate health and safety risk to children in care.
Based on interviews, photos and record review S4 had no ECE units but was being used to supervise children in the kitchen and classroom. This poses a potential health, safety or personal rights risk to persons in care.
Based on observations/interviews, the licensee did not ensure the personal rights of children in care in that 17 of 18children in care (C1 – C17) did not wear face coverings while in the facility, as required by the Order of the State Public Health Officer (June 11, 2021), and an individual face covering exception did not apply. This is a potential health and safety risk to children in care.
Based on interviews and photo’s the license had infants commingled with the preschool children which is beyond the license’s conditions of 2 YOA to entrance into first grade. This poses an immediate health, safety or personal rights risk to persons in care. 1 The director will develop a written plan of operation that includes not commingling ages beyond the capacity of their license and to include the process for requesting age exceptions if warranted. Based on interviews children who have behaviors miss whole or partial meals and are forced to nap, C3 was denied a meal and C1 was squirted with a water gun by staff due to a behavior resulting in the child crying and staff have been witnessed to make inappropriate comments, using mean tones and in a bullying manner. This poses an immediate health, safety or personal rights risk to persons in care. 8 The plan and training document will be submitted to CCL by 12/3/21.
Based on interviews S2 restrained C1 on 3-4 occasions, for 2-4 minutes. This poses an immediate health, safety or personal rights risk to persons in care. The director agrees to have all staff watch the CCL Child Care Personal Rights Videos. A signed document will list the date of training, names of attendees and topics covered. The director will also develop a plan on how a child with behaviors will be dealt with which does not include placing children in restraining holds or devices. The plan and training document will be submitted to CCL by 12/3/21.
Based on interviews school aged children were asked to volunteer at nap times to rub the preschool children’s backs. 1 The director agrees to develop a written plan of operation regarding nap time and a staff schedule showing adequate coverage to include staff available for back rubbing until children are calmed down and/or sleeping. The written plan and staff schedule will be submitted to CCL by 12/3/21.
