Based on observation and interview, the licensee did not comply with the section cited above as LPA observed a toilet in one of the preschool classrooms to be loose and not secured as it was able to be rocked when pushed and LPA also observed that the awning above the play structure had a broken pole hanging from above which poses a potential health, safety or personal rights risk to persons in care.
Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that sign in/out sheets did not all have full signatures from authorized representatives which poses a potential health, safety or personal rights risk to persons in care.
Based on observation and interview, the licensee did not comply with the section cited above as LPA observed a opened gallon of 2% milk in the kitchen fridge and Licensee stated that the 2% milk was served to children due to being unable to find 1% milk at the store which poses a potential health, safety or personal rights risk to persons in care.
