Based on Facility's report received on 02/10/2026, a lack of supervision for C#1 occured. This poses a potential risk to the health, safety and personal rights of children in care. *** This is an amended report. ***
Director to conduct a training for staff and provide documentation of training. Based off of report sent in by facility and interviews conducted it was found that three children were left in the bathroom for approximately less than 60 seconds. This poses an imediate health and safety risk to children in care.
