Based on interview and record review, the licensee did not comply with the section cited above. Facility did not have a designated director. Mr. Ilaian has designated himself as acting director when his director left a few months ago. Mr. Ilaian does not qualify as a director. He is missing units in Administration. Based on observation and record review, the licensee did not comply with the section cited above. LPA observed 17 preschool children supervised by Mr. Ilaian (Fully qualified teacher), Ashley Ward (Aide) and Fariha Amin (Aide). This deficiency poses a potential health, safety or personal rights risk to persons in care.
Based on interviews conducted by LPA, infants under 1 year old and preschool age were allowed to use Infant Room 2 during drop off times. This poses a potential health, safety or personal rights risk to the children in care.
Based on interviews and record review, the licensee did not make sure that 2 out of 2 preschool staff have full teacher qualifications before employment and they had children in care while no owner or director on site, which poses an immediate Health, Safety, or Personal Rights risk to the children in care.
Based on video footage taken on 4/24/23 in the preschool classrooms children were awake on cots in the napping area. The children were not provided an alternate activity to do outside of the napping area during what was observed to be napping time. This poses a potential health, safety or personal rights risks to the children in care.
