Based on observation, interview, and record review. The licensees did not comply with the section cited above. At 9:00am, LPA reviewed staffs file, Staff #2 (C2), and Staff #3 (S3) were missing Immunization Record proof of influenza, pertussis, and measles on file, which posed a potential health, safety or personal rights risk to persons in care.

At 9:00am, LPA reviewed staffs file, Staff #2 (C2), and Staff #3 (S3) were missing a photocopy of the teacher's transcripts, which posed a potential health, safety or personal rights risk to persons in care.

Based on observation, interview, record review, the licensee did not comply with the section cited above, 1 out of 3 medications in storage was expired, which poses a potential health, safety or personal rights risk to persons in care.

Based on record review and interview, Martha Rios (S1) worked at the facility without criminal clearance since May 2021; S1 did not come from June to August, 2021, and return back on Sept 6, 2021.The facility failed to obtain a california clearance. This poses an immediate Health and Safety risk to the children in care.

(a) The licensee shall ensure that personnel records are maintained on the licensee, administrator and each employee. Each personnel record shall contain the following information: (11) A health screening as specified in Section 101216(g).This requirement is not met: 1 Director stated that she will send in the health screening for S1 by Oct 29, 2021. Based on record review, S1 is missing health screening. This poses a potential Health and Safety risk to the children in care. 8

Base on record review, S1 is missing Mandated reporter training. This poses a potential Health and Safety risk to the children in care. 8

(a) (1) Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year. 1 Director stated that she will send in the immunization card for S1 by Oct 29, 2021. Base on record review, S1 is missing her immunization card showing that she has been immunization against influenza, pertussis, and measles or not. This poses a potential Health and Safety risk to the children in care. 8
