Based on interviews and record review, the licensee did not comply with the section cited above where as S1 made inappropirate physical contact with a child in care on or about 01/15/2025, which posed an immediate Health, Safety, and Personal Rights risk to children in care. and will notify the Department of the Districts protocols. ***This is an amended report that was created on 08/29/2025.
Based on observation, the licensee did not comply with the section cited above in not having a carbon monoxide detector at the facillty, which poses a potential health, safety or personal rights risk to persons in care.
