Based on video observations, & interviews, the licensee did 1 Facility Director agrees to provide a written statement on how they will ensure that staff have been provided training and full understanding of any form of discipline or punishment is not allowed. not comply with the section cited after S1 is observed restraining C1 on the floor, holding their wrists, setting C1 on the nap mat forcefully, & attempting to hold C1 down. S1 appears to hit C1 in retaliation after C1 bites S1, which poses an immediate health, safety, or personal rights risk to children in care. 8 Director also agrees to have staff view the video provided by Department's website ( https://ccld.childcarevideos.org ) regarding Discipline. The plan of correction (POC) shall be submitted to CCLD on or before 8/29/2025.

Buildings and Grounds -The child care center shall be clean, safe, sanitary, and in good repair at all times to ensure the safety & well-being of children, and employees, and visitors. The licensee shall take measures to keep the center free of flies, other insects, and rodents. This requirement is not met as 1 Facility Director agrees to provide a plan of correction to an alternate method other than a private party to maintain the facility free of roaches, and other insects, etc. The plan of correction shall be submitted to CCLD on or before 9/27/2025. evidenced by: Based on observations, and interviews, the licensee did not comply with the section cited where roaches were observed during the facilities hours of operation while children were in care, which poses a potential health, safety, or personal rights risk to persons in care. 8

(a)The licensee shall provide care and supervision as necessary to meet the children's needs.(1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. Extra training with staff about proper head counts and have staff zoned out in the play area and will submit a plan of correction to CCLD by 7/10/25 This was not met as evidence by: based on interviews, staff did not do a head count and iit could not be determined how long C1 was outside left unsupervised on the playground while class was transitioning from outside to inside. This poses an immediate health, safety, or personal rights risk to persons in care. Will have parents sign LIC 9224 and will submit to CCLD by 7/10/25

Based on interviews an unfingerprinted adult was providing care to children on numerous occassions. An immediate civil penalty of $500 applies. This poses an immediate health and safety risk to the children in care.
(a) There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance, except as specified in (b) and (c) below. 1 Licensee/director will ensure that they have met the teacher ratio requirements with qualified staff and adjust scheduling for staff. Plan of correction to be submitted to CCL by 8/18/23. This regulation was was met as determined by: Staff interviews indicated teacher was often left alone with as many as 15 children. This poses an immediate risk to children in care. 8
(a)The licensee shall ensure that each child is accorded the following personal rights: (7)Not to be placed in any restraining device. Postural supports may be used as specified in Section 101223.1. 1 Facility removed chair with buckle on 4/6/23. A mandatory staff meeting to go over child safety, licensing requirements, etc. is scheduled for 4/10/23.Facility to provide sign in sheet and agenda to CCL by 4/12/23.
