Based on UIR review by LPA Lujan second confirmed outbreak occurred 6/10/26. Director reported outbreak on 07/03/2026 via email on an LIC624. Facility did not met required reporting requirement in a timely manner to report. 8 website link on Reporting Requirement video. Per Director, she will write a letter on what they learned and will submit it to LPA Lujan on 08/06/2026. Per Director, she also submitted LIC624 to the department on 07/03/2026.

Based on record review, the licensee did not comply with the section cited above in 5 out of 7 staff did not have the required immunization records for mesasles, pertussis, and tuberculosis which poses a potential health, safety, or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in 25 out of 42 children were not signed in at the time of inspection which poses a potential health, safety or personal rights risk to persons in care,

Per Director, she was not aware of the reporting requirements. Director was given the reporting requirements. Director wrote a declaration that states that she is aware of the reporting requirements. Staff did not follow the required protocol in terms of reporting requirements as the 2/22/23 incident was not reported to Child Care Licensing within the required 24 hours.

Based on record review, the licensee did not comply with the section cited above: LPA observed Director's CPR expired in 2021 which poses/posed a potential health, safety or personal rights risk to persons in care.
