Based on record review, the licensee did not comply with the section cited above in classroom #4 (toddler component) where children under 24 months did not have sleep logs available for review which poses a potential health, safety or personal rights risk to persons in care.
A licensee shall not operate a child care center beyond the conditions and limitations specified on the license, including the capacity limitation. This requirement is not met as evidence by: Director stated that she will no longer use the area until it gets approved by the fire department and Licensing. Director will submit a written plan of correction to LPA Siao by 03/23/2023. LPAs Siao and Clenista observed the day care children using the upstairs multipurpse room "Beit Midrash" for PE due to the rain. The area is not an approved area for the children in care. This poses a potential health and safety risk to children in care. 8
Based on water testing results and interviews, facility tested over the Lead Exceedance level at 2 sink faucet measuring one at 6.9ppb and the other at 11.0 ppb. Director stated the faucet has been used to fill filtered Brita water pitchers and provided the water to children in the past. This poses an immediate/potential health, safety or personal rights risk to children in care.
