Based on record review, the licensee did not comply with the section cited above by testing after the deadline of 1/1/2023, which posed a potential health, safety or personal rights risk to persons in care.

Director stated she was unaware that staff was not associated. Staff was removed, and shall not return until she has been associated. At 11am During walk through of facility, LPA observed that there was an unasociated individual working in the facility. During conversation with indivdual, individual stated she has been at facility for a week.This poses an immediate health and safety risk to children in care.

Director states she will complete files by 10/29/2021. During staff file review it was observed that there were not any licensing documents as well as documentation of transcripts. This poses a potential health and safety risk to children.
