The roll sheets must be verified to ensure it matched the physical count. POC requirement has been met. Teacher failed to provide the required level of supervision. A child was left unsupervised on the playground. This lack of supervision poses an immediate health, safety, and personal rights risk to children in care. Children shall be supervised at all times, including continuous visual observation. 8 This is an amended report.

Director failed to notify the department with the by the next working day which pose a potential health and safety risk to children in care. And must submit a written report containing all information required under subsection (d)(2) within seven days. The facility will implement corrective measures to ensure all future incidents are reported within the required timeframes and that staff are retrained on proper reporting procedures.
