As a plan of correction, principal provided LPA today with a resource handout to reinforce requirements for care and supervision, for all staff. Per principal, all staff will be required to read and sign hand out and watch the, "Supervision children in childcare center" video on the CDSS online resource webpage. Principal will provide LPA with a copy of staff acknowledgement signature forms. Per principal, staff have also been designated to take their breaks when the class is inside. Staff no longer take breaks when the class is outside, all staff are required to be present outside during outdoor playtime. LPA received a copy of the staff outdoor designated area for supervision. During the investigation and based of staff interviews C1 sustained an injury and no adult staff observed what occurred, which poses an immediate health, safety, and personal rights risk to children in care.

Based on observation, interview, record review, the licensee did not comply with the section cited above due to Staff#4 not having proof of the AB1207 Mandated Reporter Training on file, which poses a potential health, safety or personal rights risk to persons in care.

Staff did not ensure adequate supervision was provided resulting in child sustaining an injury while in care
