Based on observation, the licensee did not comply nwith the section cited above. LPA observed a climbing structure in the front yard that is 4'5" high with a slide that is attached. Rubber mattings (2 - 1" rubber mattings were placed around the fall zone. Insufficient cushioning around climbing structure poses a potential health, safety or personal rights risk to persons in care.
Based on record review, the licensee did not comply with the section cited above. Staff Hardeep Hunjan did not have a copy of health screening on file. This poses a potential health, safety or personal rights risk to persons in care.
