(a) There shall be a ratio one one teacher visually observing aldn supervising no more than 12 children in attendence... This requirement was not met as evidence by: 1 Licensee will train staff and submit a copy of the agenda with staff's signatures to LPA Vang via email. Based on LPA's observation and interviews conducted, S2 went inside the facility to use the restroom and ended up helping a child throwing up, while S1 was left alone out of ratio outside. S1 was supervising 16 children. This is a potential risk to the health and safety of children in care. 8
