A licensee shall not operate a child care center beyond the conditions and limitations specified on the license, including the capacity limitation. This requirement is not met at evidenced by: LPA observed Facility Representative must provide a plan of correction to ensure that commingling of children stops occuring. POC is due by 02/28/2022. one infant and ten preschool age children on the playground supervised by Facility Representative Manley. This is an immidiate risk to the health and safety of children in care. 8

There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance, except as specified in (b) and (c) below. 1 Facility owner will submit a written plan of correction to address ratio issue. The plan of correction letter is due 02/28/22. LPA will return to correct the violation. This requirement is not met as evidenced: LPA observed fourteen children supervised by one staff on the outdoor playground. 8
