Based on observation and record review, the licensee did not comply with the section cited above in facility is not conducting 15 minute sleep observation log for children 24 months and younger which poses/posed a potential health, safety or personal rights risk to persons in care.
On 5/17/2024 Director removed worn bikes from the facility. On 7/11/24, LPA observed the bikes to be in good repair and good condition. On 5/17/24, LPA Garcia inspected and observed bikes to be in worn condition and repair which poses a potential risk to children in care.
Based on observation, the licensee did not comply with the section cited above in the cushining for the swings is worned out and can cause tripping hazards which poses/posed a potential health, safety or personal rights risk to persons in care.
