Based on LPA Susan Brewer's review of staff records, the licensee did not comply with the section cited above, Director was missing Proof of MMR, TB screening and Influenza vaccination or letter to decline, Staff 2 was missing immunization Records for all required vaccinations, Staff 3 was missing Influenza vaccination or letter to decline; which poses a potential health, safety or personal rights risk to persons in care.

Based on LPA Susan Brewer's record review, the licensee Monica Jimenez, did not comply with the section cited above in facility Staff did not complete the California Child Abuse Mandated Reporter Part 1 or Part 2 Training, which poses a potential health, safety or personal rights risk to persons in care.

Based on LPA Susan Brewer's record review, the licensee Director Monica Jimenez, did not comply with the section cited above in the director failed to complete the requried Health and Safety trainiing and did not ensure that designated opening staff present to supervise in the absence of a director, did not complete 16 hours of health and safety training if necessary pursuant to Health and Safety Code Section 1596.866.which poses a potential health, safety or personal rights risk to persons in care.
