Based on interviews and observation, staff did not have a 360-degree view of the child, which poses/posed a potential risk to the health, safety, or personal rights of persons in care. 8 LPA observed that extra mirrors were installed on the south side wall of the partitioned middle area to provide a frontal view of the children if they turn their backs to the staff.

Through staff interviews and records reviewed, it was confirmed 1 Cassandra stated that facility will conduct a staff training on care and supervision and provide proof of training materials to LPA. Cassandra will also provide a list of all employees who attended the training along with their written statements documenting what they learned. child #1 and child #2 left the playground through a unsecured gate in the play yard. Children were found by another staff. Another incident occurred where a child was left in play yard while their class was inside, this poses an immediate Health, Safety, and/or personal rights risk to children in care. 8 Additionally, Site Supervisor will submit a letterhead statement on how staff will ensure that all children are being accounted for. Based on records reviewed & Interviews conducted, Facility did not report incidents to the parents/Guardians of children in care. Incidents were also not reported to the Fresno Regional Office. This poses a potential risk to the health, safety, or personal rights of children.
