Based on record review, the licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care. LPA did not observe current Mandated Reporter Training for Staff #1 and #2.
Per Director, additional training for all teachers will be conducted pertaining to Personal Rights. Once training is completed, facility will provide proof to the department by the POC date. Three of six staff interviewed indicate that on more than one occasion, they have witnessed S3 handle children in a rough manner.
LPA observed the ceiling in Per AD: Already called parents to pick up their toddlers, 1 toddler has been moved to the infant room, 1 has been picked up and 2 are awaiting to be picked up. Children will not occupy the classroom until the leak is fixed. An emergency work order has been placed. the Toddler room has a leak, hole in the ceiling and water dripping down into a trashcan. This poses a potential health and safety risk to the children in care. 8 Will submit photo and invoice to LPA when the ceiling is fixed.
Water testing results identified with Action Level Exceedance as defined in WD section 101700.3 are not deemed safe to drink. This
