During the facility tour the LPA observed S1 Providing Care & Supervision as a Lead Teacher along with one Aide. During the file review the LPA was unable to review transcripts with all required classes to qualify S1 as a Lead Teacher.

Responsibility for providing care and supervision: No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. Director agreed to conduct an in service training with all staff by 10/26/23 regarding the importance of care and supervision. This requirement was not met evidenced by: a child was left alone for a short period of time until they were found by a parent. Please note that this violation results in a $500 civil penalty. 8
