Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that: S2, S4, S5, and S6 were missing proof of the measles immunization; S4, S5, and S6 were missing proof of the pertussis immunization; and S5 and S6 were missing proof of the influenza immunization/declination which poses a potential health, safety or personal rights risk to persons in care.
Based on record review, interview, and observation, LPA observed 9 preschool children (3 and 4 year olds), in the Kindergarten classroom. This classroom is part of the School Age license, which poses an immediate health and safety risk to persons in care. 8 statement of understanding of regulation 101261(a). (Part 2) submit an updated LIC200A, facility sketch, and all required documents and fees associated with requesting a capacity increase for the Preschool license and/or capacity decrease for School Age license.

Based on records reviewed and interviews conducted the facility did not ensure all staff have clearances transferred and associated to the facility. This is an immediate risk to the health and safety of children in care.
Facilty has employed remediation efforts by having all wood chips and substrate removed and replaced on the preschool playground. Staff did not locate or observe any safety hazards which were later discovered by children in care. A box/carton containing multipe razor blades was discovered by children in care, resulting in two injuries. Staff stated they conducted a safety check of outdoor playspace prior to incident but did not observe any hazards. This is an immediate health safety and personal rights risk to children in care. 8 Director will conduct training with staff regarding routine safety checks of outdoor play areas, especially when work crews were know to be present and could potentially leave hazards behind. Assistant Director was reminded of their Reporting requirements and was provided a copy of Reporting Requirements regulations at time of this visist. Assistant Director will provide Licensing with a written acknowledgement and statement of understanding and adherance of these Facility made alterations and closed/prevented use of outdoor activity space for an undisclosed period of time without reporting closure or alternatives for outdoor activity space to Licensing as required. Based on observation, interviews, and record review, the licensee did not ensure that all employees were actively associated to the facility which poses an immediate Health, Safety, or Personal Rights risk to persons in care.
Based on record review, the facility did comply w/ith written directives outlined in PIN 21-21-CCP and failed to have 2 kitchen sink faucet water sources tested for safe lead levels of 5.5 ppb or below, which is a potential risk to the health and safety of daycare children.
Based on LPAs observations made of video footage recorded on 12/28/2022 at approximately 10:53 a.m., staff failed to prevent a daycare child from repeatedly pulling and yanking the hair of another daycare child, which posed an immediate risk to the health & safety of children in care. 8 The director agrees to provide additional training to child care staff and submit the plan of correction to Community Care Licensing Riverside, on or before the close of business day 01/20/2023, by fax or e-mail.
