101223 (a)(1) -Personal Rights- (a) The licensee shall ensure that each child is accorded the following personal rights: (1) To be accorded dignity in his/her personal relationships with staff and other persons. 1 Director shall submit a Plan of Correction to the Department by 4/10/26. The plan shall include the staff will watch the Personal Rights videos online. Also, a meeting with entire staff with an outside consultant by 5/4/26. In addition, an agenda will be sent with the topics as well as all staff will sign form acknowledging they attended and understand. Based on evidence and requirement was not met, LPA Wheatley interviewed several staff members who have witnessed the children being yell at and spoken to in an inappropriate manner. Also, in regards to potty training, children have been spoken to in an inappropriate manner and made to feel uncomfortable or embarrassed. This is violation poses an immediate risk to the health & safety of children in care. 8

101223(a)(3)-Peronsal Rights- To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature including but not limited to: interference with functions of daily living including eating, sleeping or toileting; or withholding of shelter, clothing, medication or aids to physical functioning.
