Based on observation, interview and record review, the licensee did not comply with the section cited above in 1 out of 3 staff which poses/posed a potential health, safety or personal rights risk to persons in care. Subject staff's Mandated Reporter Training expired on 03/25/2023.

101700.3 (b) Testing results with fractional ppb readings of 0.5 ppb or greater shall be rounded up to the nearest whole number, before comparing to the Action Level. (1) A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. During visit, LPA confirmed lead results were posted, and lead documents were submitted. Director will remove, replace or repair outlet A and will submit plan of correction by 02/03/2023. Please refer to the PIN referanced in the report for 101704 Lead Action Level Exceedance Response Requirements This requirement is not met evidenced by record review of the lab report analysis from Outlet A taken on 11/19/2022. Results indicated levels of lead in exceedance. This poses a potential risk to the health and safety of children in care. 8

Based on interview, staff did not provide supervision for one child within one minute as children were transitioning inside of the classroom. This poses a potential risk to the safety of children in care. Doumentation of the staff meeting and copies of the policy with staff initial and date, indicating they read it, were obtained on this day.
