This requirement is not met as On 03/04/2026,there was a staff meeting that went over bottle protocol. implement, will color code bottles for each child, conduct regular training's with scenarios, conduct class observations each week or every other week. Director will provide LPA with a copy of the staff meetings, obsevationsm notes take picture of color coded bottles via email. evidenced by, on 02/18/2026 Staff 1 handed Staff 2 C1 's bottle of breast milk and C2 was fed C1's bottle by mistake. C2 consumed the entire bottle of breast milk. This is an potential health and safety risk to the children in care. 8

Personal Rights: Each child shall be accorded safe, healthful and comfortable accommodations, furnishings and equipment. This requirement is not met as On 11/09/2022, The Director sent an email to all infant staff regarding feeding protocol. A copy of the email was provided to LPA. Families involved in the incident were notified evidenced by, on 11/09/2022, a Staff#1 warmed a bottle of breast milk for chil#1, a baby in the infant classroom. Staff#1 placed it on the meal cart and warmed a bottle of breast milk for Child#2 and one of our interns took the bottle from the warmer, wrapped it in a towel without looking at the name of the child, and started to feed child#1 with child#2 bottle. This is an immediate health and safety risk to the children in care. 8 immediately. A copy of the training and staff attendance was provided on 11/28/2022 to LPA Miranda. Director will submit to LPA no later than tomorrow copy of LIC9224 form sign by parent/guardian from all infants enrolled and place a copy it in each child’s file.

Lead water testing samples taken on 09/22/2022 revealed that levels of lead 1 The water outlet was immediately decommissioned and tagged for further action by the UCLA Facilities Team. UCLA Facilities will install replacement of the localized fixture to include the angle stops and supply lines and replace the hose bib. Testing will be done again once the work is completed. were beyond the allowable amount deemed by the State of California. Water Outlet “I” has Action Lead Exceedance (ALE) level of 8.3 pbb. This is a type B violation which poses a potential Health, Safety or Personal Rights risk to persons in care. 8 A copy of the re-test summary report will be emailed to LPA Martinez on or before Monday, 11/28/2022.
