Based on observation and interview, the licensee did not comply with the section cited above in 31 out of 31 persons, as LPA observed a large wooden playhouse on the playground that had several areas of wood that was in disrepair which poses a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the directive above, as small sink kitchen (water source C) used for staff hand washing tested with an Action Level Exceedance (ALE), which could pose a Health and Safety risk to children in care. 8 POC has been recived by the SDCCRO. Citation will be cleared once sink and piping have been completely replaced.
