Based on observation, interview, the licensee did not comply with the section cited above in that there is a 10in. hole on the outside wall near the ramp that leads to the entrance of toddler classroom, a post with chipped paint outside of the preschool classroom and areas without a barrier that have access to an unused bigger playground which poses a potential health, safety or personal rights risk to persons in care.
Based on observation, the licensee did not comply with the section cited above in that ceiling panels in the classrooms have water stains, with also a broken panel in the toddler room, which poses a potential health, safety or personal rights risk to persons in care.
Based on observation, the licensee did not comply with the section cited above in that a mop bucket with standing water was seen at 11:24am and 12:08pm which poses a potential health, safety or personal rights risk to persons in care.
Based on observation, the licensee did not comply with the section cited above in that a storage room containing cleaning solutions was found unlocked. Also, a cabinet within the toddler room had a cup of adult scissors on a shelf approximately 3 feet from the ground, which poses a potential health, safety or personal rights risk to persons in care.
