Based on interviews conducted, and records review, the licensee did not ensure that each child to be free from unusual punishment which poses an immediate health/safety, and/or personal rights risk to persons in care.
–(b) Result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance This requirement was not met as evidence by record review. Lead test results showed that two water sources had an action level exceedance. 1 Per facility representative, both fixtures with lead exceedance were covered with trash bags pending for the faucet to be replace and retest was scheduled for mid July. Facility will update RO with the resting result upon completion. LPA observed the sinks located in the kitchen area to be made inaccessible with plastic bags and tape pending for the faucets to be replaced and rested. This is a potential risk to the health and safety of the children in care. 8
based on record review facility failed to provide S#20 records of completion or enrolling in early childhood education or child development units. S320 is fingerprint cleared and associated to the facility 1 Facility representative indicated S#20 has been removed on 11/15/2022 and no longer working for this facility. LPA did not observed S#20 during today's visit. LPA obtained a declaration statement. LPA to dissociate S#20 from the facility. POC cleared during today's visit. which poses a potential health and safety risk to children in care.
