Based on interviews and record review, a child left the facility's classroom unattended by staff, however, was still on site due to a lack of supervision. This posed a potential Health and Safety risk to children in care.

Based on statements provided by parents and staff, and a review of the Parent Handbook 1 The Facility Representative said she the facility management would meet with school campus admission to review their plan/policies on photograph, they would create a written policy including addendum in the Parent Handoobk to updating social media & staff usage policies in accordance with CCR which confirmed the facility did not comply with it's own plan of operation. Based on LPA's review of the sign in/out procedure which revealed that multiple child were not signed out by their authorized representative. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on a review of Department records 1 LPA provided S1 with required transfer documents to request a transfer. S1 completed the forms and submitted the forms to the Department to request a transfer of her clearance. Director said in the future, she plans to review Gaurdian to ensure all staff ar cleared and associated to the facility license. which revealed S1's criminal record clearance was not associated to the facility license. Based on staff (S1-S19 & CD) records reviewed which revealed S11's record did not contain a current AB 1207 Mandated Reporter Training certificate. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on staff (S1-S19 & CD) records reviewed which revealed S1 was missing LIC 503 & 501. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on staff (S1-S19 & CD) records reviewed which revealed all teachers were missing LIC 9095. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on the interviews conducted by the LPA and staff S4's own admission on 11/01/22, it was corroborated that the facility did not notify child C1's authorized representatives of their concerns regading a suspected head injury in a timely manner. Please note: although it was later corroborated that C1 did not in fact sustain a concussion or other injury, staff S4 believed at the time that C1 did sustain a concussion. The facility should have immediately notified C1’s authorized representatives so that a medical professional could be consulted to determine the appropriate care. This posed a potential risk to the health and safety of child(ren) in care.
