Based on interviews and record review, LPAs observed an unenrolled school-aged child (C1) present in the breakroom being supervised by the Director, as the facility is licensed to care for infants ages 6wks to 2 yrs old only, which poses an potential Health, Safety, and/or Personal rights risk to persons in care. 8 Title 22 Health and Safety Code in the operation of the Child Care Center moving foward.

Based on observation and interviews, the facility was operating out of ratio on 3/18/26 when S1 was alone in a classroom with 19 children, which poses a potential health, safety or personal rights risk to persons in care. 8 adequate staffing for the Toddler 1 & 2 classrooms, no later than 6/8/26. These corrections will be submitted via email to LPA at Gerald.Poindexter@dss.ca.gov.
Based on LPA observation on 3/26/26, there were construction materials -- nails and electrical cords -- at the entrance/exit of the Toddler 2 classroom, as well as in proximity to the facility entry gate, which poses a potential health, safety or personal rights risk to persons in care.

Based on staff interviews, and review of camera footage an infant was left on top of a boppy pillow on thefloor. A blanket was left above the infant’s head and due to lack of supervision, the infant was able to grab the blanket and dragged it over the infant’s head.
Based on staff interviews, and review of camera footage, it was observed that a parent walked into the classroom and found an infant's head covered with a blanket. Video footage reviewed also revealed that the infants head was covered for approximately 45 seconds.
Based on interview and record review, site director failed to report an unusual incident via phone or fax to the department within the department's next working day. Incident that ocurred on 2/20/2025 was not reported to the department until 2/24/2025.
Based on information rec'd via interviews and document provided today, Staff #1 was observed to grab and sat a child hard on the floor and yelled at the child.
Based on LPA's observation, LPA observed Toddler #1 classroom to be out of ratio at 12:35PM. Staff #2 was observed supervising 19 napping toddlers.

It is an indicative of an inflicted injury bruise due to forceful and inappropriate handling of an infant. This poses an immediate health and safety risk to clients in care. Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours...follow up with written report within seven days 1 Director stated in the future she will be sure to contact licensing to report all unusual incidents within 24 hours and submit the written report to licensing within 7 days. Director will submit the written incident This requirement is not met as on 08/09/2023, a public agency contacted the facility and spoke with four staff members regarding a possible incident that happened to a child in care. Facility did not report the unusual incident to the licensing office. Based on record review and interview, the licensee did not comply with the section cited above. 15-minute sleep documentation is not maintained in the toddler classroom and the last 15-minute sleep documentation maintained in the Infant #2 class is dated 6/7/23. This poses a potential health, safety or personal rights risk to persons in care.
All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing or volunteering in a licensed facility. Request a transfer of a criminal record clearance as specified in Section 101170(f). Director stated that she will get a fax machine to ensure that the transfer request gets process. In the mean time she will be sure that the staff is associated prior to having the individual be in the class. This requirement is not met as evidence that staff members Ruby Thompson and Rihab Haraj's fingerprints are not associated to the facility. This poses a potential health and safety risk to children in care. 8
Based on observation and interview with Director, it was determined that although staff was physically present in the napping area at all times an infant was sleeping, they were unaware that 15 minute checks were to be documented. This is a potential hazard to the health and safety of children in care.
Based on observation and interview with the Director, none of the infants under the age of 12 months had Form LIC 9227 on file. This is a potential hazard to the health and safety of children in care.
