Based on interviews and documentation review, the Site Director states that there is no written sanitation policy or procedure or that reusable cups require water only in the parent handbook, which poses a potential risk to the Health, Safety, or Personal rights to persons in care. 8 Copies of the updated policies and procedures will be provided to parents and Licensing by 08/28/26. Also, staff will be informed of the policy and procedure updates to ensure consistent and improved communication with parents.

Based on documentation and interviews, it was determined that that a staff member recorded and posted the child in an inappropriate manner and without permission. In a separate incident, a child was left without proper clothing, this poses an immediate health, safety or personal rights risk of children in care. 8 Training was held on 4/14/2026 and all staff signed Agenda Notes. Proof of meeting notes were emailed to LPA Macias on 4/22/2026.

… Director stated they will have extra staff available to help with potty-training and bathroom assistance so that adequate supervision and visual observation is maintained at all times when children are also on the playground area. Staff will receive training notifying them of the additional staff requirement and to request additional staff when needed to maintain proper supervision. Director will send written plan of correction with staff sign-in sheets acknowledging staff training. Based upon LPAs observations and interviews with staff members, children and parents of enrolled children, licensee did not ensure staff supervision including visual observation because staff member was helping children in the bathroom which resulted in child’s injury requiring medical treatment and is an immediate health, safety and personal rights risk to children in care. This is a repeat violation. Civil penalties assessed.
