a child who was located 1 A staff meeting was held at the district regarding supervision and staff placement in the classroom to allow for broader visual observance. The program installed a ring bell in the classroom that makes a ring noise when the classroom door is pushed open from the inside. outside by a teacher returning from lunch which is an immediate health and safety risk to the children in care. 8 A ring bell was also installed on the gate leading to the parking lot that makes a ring noise in the classroom when the door is pushed open. The director will email LPA L. Habtom the staff meeting agenda to clear the plan of correction by the due date of 10/13/2025.

Based on interview, the licensee did not ensure to maintain supervion of child (C1) at all times. This poses an immediate Health, Safety, and Personal Rights risk to children in care. 8 LPA was informed that staff now have a chart to track the number of students walking to the playground. Children also use a walking rope.
