4/28/22 LIcensee was not present and Teacher Desiginated did not have access to the full operation informaiton. This poses an immediately H&S to children in care. Copies Notifification that was sent to the parents showing when facility closed from 07/21 to 03/2022. Licensee agreed to training and allow the designated person have total access the documentation while the Department come for inspection.

The Director will submit the documents to the Department by 10//15/2021.

Menus shall be made available for review by the child's authorized representative and the Department upon request. This standard was not met based on evidence obtained through the interviews and observations. 8 Food Services previously.
