Staff, S3 was observed to be missing several documents including proof of immunization's and Mandated Reporter Training.

Based on observations and interviews, it was determined that during a parent event at the end of the day, child, C1 ran out of the two facility entrance gates, outside the center and down the sidewalk parallel to the left side outdoor area. This poses an immediate risk to the health, saftey and personal rights of childrne in care.

Based on observation, the licensee did not comply with the section cited above, which poses a potential health, safety or personal rights risk to persons in care. LPA observed that there is not gate or door to make the kitchen inaccessible. There are knives in a drawer that is accessible to children.

Program Director provided they have submited a detailed plan to ensure that children are supervised at all times in the Self reported UIR. Since this is a zero-tolerance violation, an immediate civil penalty of $500 was assessed during the inspection. A child left the playground and entered the classroom with out a Teacher being present in the classroom. This is a zero tolerance violation. This poses a immediate risk to the health and safety of children in care. 8 Upon receipt, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months.
