(a) The licensee shall ensure that each child is accorded the following personal rights: To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule....... This requirement is not met by evidence by: 1 Ensure it does not happen again and children will continue to be supervised per school protocol and state mandate. Create a map of the play ground and teachers areas stationed to see the children at all time. Starting immediately. Based on observation and interviews, the licensee did not comply with the section sited above in that C1's pants were pulled down, exposing his private parts, by C2 while they were hanging on the monkey bars, which posed a health, safety or personal risk to children in care. 8 Under Appeal

Supervision shall include visual observation. This requirement is not met by evidence by: 1 Ensure it does not happen again and children will continue to be supervised per school protocol and state mandate. Create a map of the play ground and teachers areas stationed to see the children at all time. Starting immediately. Based on observation and interviews, the licensee did not comply with the section sited above in that C1 was allowed to lock C2 in a shed until Staff #2 came from the classroom and observed C2 standing in front of the shed and heard C1 knocking from inside. which posed a health, safety or personal risk to children in care. 8

Create a map of the play ground and teachers ares stationed to see the children at all time. Starting immediatly. Based on observation and interviews, the licensee did not comply with the section sited above in that C2 locked C1 in an unlock shed on the playground, by holding the door close until staff let C1 out which posed a health, safety or personal risk to children in care. 8

This requirement is not met by evidence by: 1 Incident report training with all staff. Incident must given to parents, the same day for signature and the awareness to head of school. This will start when school resumes 8/28/24. Signed training document will be sent to LPA via email by 9/15/24. Based on record review, the licensee did not ensure William Acher Biggs Chiropolos (date of hire 9/27/23) was associated to the facility which poses a potenial health, safety, or personal rights risk to persons in care. A civil penalty of $500 has been assessed. 8
