Based on LPA's observations staff did not ensure that child #1's medication was current and not expired. Expiration date: 05/2023. This poses a potential health and safety risk to children in care.

Based on LPA observation and record review, the facility did not have complete education requirements available on file for one of four staff. Angela Richter’s full credentials were not available at the time of inspection.

Based on statements by witnesses, staff was observed removing a child's shirt when child failed to follow staff's request to stop putting her hands in her shirt and lifting her shirt up. Witnesses observed child crying and covering herself with her arms. 8 Anger Management; Conflict Resolution Strategies; Guiding Children's Behavior in Childcare Settings; Behavior Management for Young Children and Positive Solutions for Challenging Behaviors. Proof of course completion to be submitted to LPA no later than March 30, 2022.
