based on video footage, Licensing Program Analyst (LPA) observed one teacher aggressively or roughly handling two (2) children during nap time. This poses a potential risk to the health, safety, or personal rights of children in care.
Based on record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. During employee record review, Licensing Program Analyst (LPA) observed that two staff members were missing Mandated Reporter Training for Child Care Providers.
Based on observation, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. During the indoor inspection, Licensing Program Analyst (LPA) observed dark staining in different areas of the brown carpet as well as staining on the colorful carpet used for circle time in the 4s classroom. LPA also observed several holes and frayed areas in the brown carpet. During the outdoor inspection, LPA observed that one of the wooden boards toward the bottom of the fence was broken, leaving a splintered area.

During staff records review, Licensing Program Analyst (LPA) observed that staff #5 was missing immunization records for Measles, Pertussis, and influenza. This poses a potential risk to the health, safety, and personal rights of children in care.
